Extended Winter Opening Hours 2026

The practice will be open from 8.30am until 4.30pm on the following dates for urgent and emergency care only.

Please call the surgery if you require urgent care that cannot wait until the next working day.

Please note, we will be unable to deal with routine or non-urgent matters until the surgery reopens on the Monday.

If you require medical care or advice at any time when we are closed, please call 111.

Annual Health Review for Patients with Chronic Conditions

At Portland Park Medical Practice, we are committed to ensuring that our patients with chronic conditions, such as Asthma, COPD, or Diabetes, receive the ongoing care and support they need. To help manage these conditions effectively and maintain your health, we will invite you for an Annual Health Review during your birthday month.

What is the Annual Health Review?

The Annual Health Review is an important check-up designed to monitor and manage your condition. During this review, we will assess how well your treatment is working, discuss any changes in your health, and provide guidance to help you manage your condition more effectively.

How will you be notified?

You will receive an invitation to book your Annual Health Review by text message or letter. This invitation will include details about your patient group (e.g., Asthma, COPD, Diabetes) and instructions on how to schedule your appointment.

What should you do?

  • Please wait for your invitation: You do not need to call the practice until you receive your invitation. Once you have received it, you can contact us to book your appointment at a time that suits you.
  • Get in touch when invited.  If you have not heard from us yet, please be patient. We will reach out to you directly.
